Inclusion criteria

Inclusion criteria: 1. The registered address and current address are the objects of Xuhui District, Shanghai; 2. Aged >=75 years; 3. The elderly living at home; 4. The elderly with weak cognition: the elderly with weak body (including the early stage of physical weakness) and cognitive function decline, in which the standard of judging physical weakness (including the early stage of physical weakness) is at least one positive item in the revised fried pf score, and the standard of judging cognitive function decline is MMSE score lower than normal or subjective complaint memory decline; 5. The elderly with informed consent and voluntary participation in the study.

Exclusion criteria

Exclusion criteria: 1. Old people who cannot communicate normally; 2. Old people who are completely disabled; 3. Patients with advanced cancer, dementia, severe organ failure, etc; 4. The elderly who clearly expressed their unwillingness to participate in the follow-up formal intervention activities one week before the formal intervention (the same as the content, form and time of the formal intervention activities) ended; 5. The elderly who plan to be hospitalized, stay in a nursing institution or nursing home, move or participate in other organized sports training within the next 4 months.
