38 Years to 75 Years

Inclusion criteria

Inclusion criteria: 1. meet WHO1999 T2DM diagnostic criteria and confirmed by doctors; 2. ages 18-75 years; 3. no communication barriers; 4. able to read and write in Chinese; 5. residence registered in the community health service station, whose residence time was more than 1 year; 6. Volunteer to participate in this study.

Exclusion criteria

Exclusion criteria: 1. with severe complications (hyperplastic retinopathy, nephropathy in period IV or more, cardiac function level III or more, and diabetic foot level 1 or more; 2. with other serious chronic diseases such as stroke, malignant tumors; 3. have a long-term outside living plan within nearly half a year.
