Inclusion criteria
Inclusion criteria: 1. Essential hypertension,and the systolic pressure was measured greater than 140 at 3 times in a row in doctor's office; 2. Permanent residents of the community; 3. Signing and willing to participate in this project, and giving consent to accept the diagnosis and treatment of hypertension at the designated medical institutions, and re-referred to superior medical institutions when necessary; 4. Will not drop out due to the long-term leave, going abroad or other reasons during the 1 year. 5. Aged 40-80 years; 6. Have smartphone and can use it in order to be followed by doctors; 7. The score of morisky is <= 7.
Exclusion criteria
Exclusion criteria: 1. Hypertension emergency; 2. With type 1 diabetes; 3. Type 2 diabetes mellitus with acute metabolic disorder; 4. Secondary dyslipidemia patients (secondary to patients with type 2 diabetes not included); 5. Acute myocardial infarction (<3 months) and stroke (<3 months) acute phase; 6. The history of myocardial infarction; 7. Patients with chronic renal disease (eGFR <60ml/min/1.73m2 or urine albumin/creatinine ratio = 30); 8. Pregnant women and nursing mothers; 9. Difficult to cooperate (psychiatric illness, hearing difficulty, or physical incapacitation); 10. To combine serious diseases with a life expectancy of less than one year; 11. Healthcare provider.
