Inclusion criteria

Inclusion criteria: 18-65 years of age; meeting the relevant diagnostic criteria of the Chinese guidelines for the prevention and treatment of type 2 diabetes mellitus (2017 edition) and the Chinese guidelines for the management of hypertension; adult patients who received pharmaceutical care during the hospital stay, and were diagnosed with T2DM and hypertension at discharge; receivinghypoglycemic and antihypertensive drugs.

Exclusion criteria

Exclusion criteria: medical history of acute cardiovascular and cerebrovascular diseases and other stress states; pregnant or lactating women.
