Age
65 Years to 110 Years
Inclusion criteria
Inclusion criteria: 1. The newly admitted patient >=65 years old; 2. Patients or their relatives are willing to participate in this study.
Exclusion criteria
Exclusion criteria: 1. Patients or their relatives unwilling to participate in this study; 2. Patients with less than 3 days in hospital; 3. Patients suffered from a serious disease for which death was considered imminent(<=3 months) and inevitable such as end stage organ failure, advanced cancer with multiple distant metastases or cachexia; 4. Patients suffered from severe cognitive impairment or advanced neurodegenerative disease who cannot recover or return to normal life; 5. Patients who have already take part in the study admit to hospital again in the study period.
