Intervention group:An intravenous injection of methylprednisolone 40mg wil be given 4h before extubation
control group:placebo wil be given 4h before extubation

Inclusion criteria
Inclusion criteria: 1. Aged 18-80 years; 2. Admitted into the Emergency Department and intubated for one of the following conditions: (1)Severe traumatic injury; (2)Glasgow Coma Scale (GCS) score < 8; (3)hypoventilation; (4)persistent hypoxemia (SaO2 <= 90%) despite supplemental oxygen; (5)persistent hypotension despite vasopressor treatment, or (6)cardiac arrest.
Exclusion criteria
Exclusion criteria: 1.Pregnant or breastfeeding; 2.Chronically treated with corticosteroids or other anti-inflammatory drugs; 3.Extubated for patient comfort or family request; 4.Unplanned or self-extubation (necessitating immediate reintubation); 5.Vocal cord dysfunction; 6.Deep sedation (defined as a Richmond Agitation-Sedation Scale (RASS) score <= -4); 7.Diagnosed with gastrointestinal hemorrhage within the past three months.
