Inclusion criteria
Inclusion criteria: (1) conform to the standard diagnosis of chronic heart failure, and New York heart association (NYHA) cardiac function class II - III; (2) between 18 and 80 years old; (3) living in the research community; (4) can use the telephone or the mobile phone.
Exclusion criteria
Exclusion criteria: (1) high risk for heart failure, cardiac function ? level; (2) the present state of very weak health; (3) serious mental illness or cognitive impairment; (4) the end of cancer; (5) illiterate; (6) Hearing impairment; (7) Drug or alcohol abuse; (8) have had heart surgery in the last six weeks; (9) Being or have been involved in other heart failure health management projects or similar projects.
