Inclusion criteria

Inclusion criteria: ARDS diagnosis refers to the Berlin standard, including: 1) onset of illness within 1 week, or new or worsening respiratory symptoms; 2) can not completely explain the blurred lungs by exudation, atelectasis or nodules; 3) can not Respiratory failure explained entirely by heart failure or volume overload; 4) PaO2/FiO2= 5cmH2O. (1) High-flow oxygen therapy and non-invasive ventilation in primary hospitals for patients with mild ARDS standard constrain: 1) the conditions for NPPV application:1)Have strict monitoring conditions of ICU, and can immediately perform tracheal intubation and invasive positive pressure ventilation in case of worsening condition; 2) Patients do not have organ failure; 3) SAPS II score = 7.3) can also be used with caution, but be prepared for replacement with NPPV or tracheal intubation for invasive positive pressure ventilation (2) ECMO combined with lung protection ventilation strategy for patients with severe ARDS Inclusion criteria, when severe ARDS patients could be considered to satisfy the following conditions of ECMO: using lung and joint protective ventilation lung recruitment, prone positioning other treatment, in pure oxygen,PaO2 / Fi02 600 mmHg; ventilation frequency > 35 times/min,pH 30 cmH2O; aged < 65 years; mechanical ventilation time < 7~10 d; no anticoagulation contraindications.

Exclusion criteria

Exclusion criteria: High-flow oxygen therapy and non-invasive ventilation in primary hospitals for patients with mild ARDS Exclusion criteria: 1) upper gastrointestinal perforation, upper gastrointestinal surgery; 2) Patients who cannot breathe spontaneously,such as severe craniocerebral injury, high paraplegia, severe neuropathy, severe respiratory center depression caused by sedation or brain injury, severe electrolyte imbalance inhibiting phrenic nerve and neuromuscular junction lesions)pulmonary basis Disease ( pulmonary bullae, pneumothorax) and so on. The contraindications for HFNC are cardiac arrest, severe type I respiratory failure, moderate to severe respiratory acidosis hypercapnia (pH 7d; aged > 60 years.
