Inclusion criteria
Inclusion criteria: 1. Patients taking 5 or more drugs at the same time; 2. Patients who have recently received prescriptions from different physicians; 3. Patients with recent abnormal laboratory index values caused or exacerbated by drug treatment; 4. Patients who have recently experienced an adverse drug reaction or event; 5. Patients with low compliance and need further guidance to ensure correct medication.
Exclusion criteria
Exclusion criteria: 1. Patients with type I diabetes; 2. Patients with type 2 diabetes who are taking less than 5 drugs at the same time; 3. Patients who are pregnant or breastfeeding; 4. Patients with deafness or mental retardation that affect the quality of follow-up; 5. People with cancer, blood system diseases patient.
