Inclusion criteria

Inclusion criteria: 1. The patient was diagnosed with stroke (confirmed by head CT or MRI); 2. patients who stayed at home after discharge or continued to be hospitalized in the rehabilitation department after discharge from neurology; 3. Patients living in Youjiang District, Baise City; 4. Patients or caregivers have a smartphone and can be easily connected to the Internet.

Exclusion criteria

Exclusion criteria: 1. with serious heart, brain, lung, liver, kidney and other important organ diseases; 2. patients with intestinal organic disease, such as megacolon, anal fistula; 3. Patients who refused to receive home visits and assessments.
