Inclusion criteria

Inclusion criteria: Inclusion Criteria of patients We will identify eligible patients via patients medical records in the village clinic. The inclusion criteria for the patients who are: (1) having essential hypertension without adequate BP control (2) aged 18 years or above; (3) living with their family. Inclusion Criteria of family carers Each patient nominated one family carer. The inclusion criteria for family carers are: (1) aged 18 years or above; (2) a member of one family with kinship, marital, or co-residence relationship; (3) able to be contacted by telephone or WeChat.

Exclusion criteria

Exclusion criteria: The exclusion criteria of the patients are those who are: (1) diagnosed with terminal illnesses (e.g., cancer, end-stage renal disease, severe heart failure, etc.) (2) with history of mental disorder, psychiatric illness (e.g., dementia, and schizophrenia); (3) having comorbidity of any other medical diseases (such as stroke, COPD, and myocardial infarction); (4) Participating or having been participated recently in a hypertension management program in the last 6 months, by asking the patients with a question of Have you joined any hypertension management program in the last 6 months? The exclusion criteria for family carers include: (1) Those with a severe mental illness (e.g., dementia, schizophrenia, and manic depression) and/or learning disorder, as reported by the family carer. (2) Those are caring for more than one family member with acute or chronic illness.
