Inclusion criteria

Inclusion criteria: 1. Diagnosed type 2 diabetes, the most recent one glycated hemoglobin values 8% or more (12 months); 2. Shenzhen city resident population, can check to the hospital in time during the study period. 3. Aged >= 18 years, can communicate in mandarin; 4. Agree to participate in this study.

Exclusion criteria

Exclusion criteria: 1. Severe heart, liver and kidney disease, advanced tumors or life expectancy less than 12 months; 2. Mental illness and disturbance of consciousness.
