Inclusion criteria

Inclusion criteria: 1. Consciousness, normal thinking, and ability to communicate in language; 2. Patients who meet the diagnostic criteria for chronic obstructive pulmonary disease revised by the Chinese Medical Association Respiratory Diseases Committee in 2013 and are allowed to leave the hospital; 3. Patients who have obtained informed consent and volunteered to participate in the intervention program; 4. aged 18 to 70 years old; 5. living community has a specialist nursing alliance hospital.

Exclusion criteria

Exclusion criteria: 1. patients with a history of mental illness and disturbance of consciousness, chronic respiratory failure, pulmonary encephalopathy and other serious complications; 2. with lung cancer, tuberculosis; 3. need to continue non-invasive or invasive ventilation, hemodynamic instability.
