Inclusion criteria

Inclusion criteria: 1. Aged >= 60 years and living in pension institution; 2. Have at least one chronic disease; 3. 2 or more daily medications (both prescription and over-the-counter); 4. Be aware and communicate with staff without any difficulty.

Exclusion criteria

Exclusion criteria: 1. Mental or cognitive impairment; 2. Unwilling to accept this study; 3. Patients with severe or terminal illness.
