Inclusion criteria
Inclusion criteria: (1) aged 60 years old or over; (2) having at least three chronic conditions of the 38 chronic diseases; (3) having at least one chronic prescription; (4) independently managing their medications; (5) capable to provide a written informed consent to participate in the study.
Exclusion criteria
Exclusion criteria: (1) being institutionalized in a nursing home or in any other long-term care facility; (2) currently participating in research involving chronic diseases management; (3) having severe mental diseases (e.g. schizophrenia, dementia, major depression) or deafness.
