Inclusion criteria
Inclusion criteria: Inclusion criteria for patients: 1.aged 70 years or older; 2.with one or more kind of chronic diseases, meet the corresponding diagnostic criteria for chronic diseases and have been diagnosed in hospitals above grade ii; 3.the estimated survival time is more than 6 months; 4.having at least one family member as the primary carer; 5.permanent residents of Wuhan; 6.volunteer for this study. Inclusion criteria for family members: 1.one of the primary caregivers of the patient (living with the patient or visiting the patient at least twice a week in the past month); 2.volunteer to participate in this study.
Exclusion criteria
Exclusion criteria: Exclusion criteria for patients: 1.unconscious; 2.in the stage of disease aggravation or terminal stage; 3.with visual, auditory and cognitive impairment and cannot communicate well with others; 4.unable to cooperate with the researcher due to mental disorder; 5.who are participating in or have participated in a similar study in the past six months. Exclusion criteria for family members: 1.with visual, auditory and cognitive impairment and cannot communicate well with others; 2.unable to cooperate with the researcher due to mental disorder; 3.those hospitalized during the study period; 4.nursing workers and other salaried caregivers; 5.who are participating in or have participated in a similar study in the past six months.
