Inclusion criteria

Inclusion criteria: The frail old people cared by the technical extension pilot unit and the frail old people cared by the technical extension unit in the same city. Frailty diagnostic criteria were adopted; Fried frailty scale.

Exclusion criteria

Exclusion criteria: 1) unwilling to participate in this study; 2) to leave the study area to live; 3) old people with a life expectancy of less than one year.
