pharmacists intervention group:1. Conduct medication education for patients taking NOAC, including drug storage, indications, adverse reactions, drug interactions, etc.

Inclusion criteria

Inclusion criteria: 1 Aged >= 18 years; 2 Diagnosis of atrial fibrillation or venous thrombosis; 3 NOAC anticoagulation therapy lasts at least one month.

Exclusion criteria

Exclusion criteria: 1 Cognitive impairment; 2 Severe hepatic and renal insufficiency; 3 Severe heart failure; 4 Experienced severe bleeding complications (such as cerebral hemorrhage and gastrointestinal bleeding) in the past three months; 5 Unwilling to receive follow-up visits.
