Inclusion criteria

Inclusion criteria: Inclusion criteria of nurses: 1. registered nurses (RN); 2. a minimum of 1 year of intensive care nursing experience Inclusion criteria of patients: 1. aged 18 years or older; 2. with an expected ICU stay of at least 48 h.

Exclusion criteria

Exclusion criteria: Exclusion criteria of nurses: 1. reject to participate in this study; 2. Unable to keep to continuity take party in the study. Exclusion criteria of patients: 1. patients with language barrier lead to inability to communicate in the study period; 2. patients who have persistent coma (RASS = -4 or -5) during the whole study which could not be assessed for delirium; 3. participating in other study of delirium interventions; 4. patients who have contraindications of intervention, including active bleeding, spinal cord injury, open lumbar drainage, and extracranial ventricular drainage; 5. patients or legal representative do not provide consent.
