18 Years to 80 Years
Inclusion criteria
Inclusion criteria: 1. Patients aged 18-80 (inclusive) years with type 2 diabetes who can visit the endocrine Department of six research centers(= 3 times a year) within the next three years; 2. Voluntary signature of informed consent; 3. Patients willing to cooperate with outpatient diabetes management.
Exclusion criteria
Exclusion criteria: 1. Organ failure or other serious diseases, including myocardial infarction occurring within 12 months before entering the group, severe mental disorders, severe infections, diffuse intravascular coagulation, etc.; 2. Active or inactive malignant tumors with a life expectancy of less than 1 year; 3. Patients with communication difficulties who can not fully understand or cooperate; 4. Patients who have accepted continuous management based on multi-disciplinary team by phones/Apps; 5. Pregnant women, lactating women and women who have had conception plans in the past year.
