Inclusion criteria

Inclusion criteria: 1. aged =35 years old; 2. inhabitants lived in the community more than 6 months; 3. under the management in the community health centers; 4. with normal cognitive function; 5. sign the informed consent and willing to participate in the study.

Exclusion criteria

Exclusion criteria: Suffering from serious mental and can not complete the survey smoothly.
