Inclusion criteria

Inclusion criteria: (1) Patients who have been diagnosed with hypertension, diabetes, hypertension and diabetes; (2) Patients who have a mobile phone for independent use and support bluetooth function; (3) Patients who live in this community for more than 6 months and have no relocation plan within 2 years; (4) Patients who are capable of reading and writing; (5) Patients who are informed consent of this study.

Exclusion criteria

Exclusion criteria: (1) Patients who under the age of 50; (2) Patients who suffering from serious complications and unable to live independently; (3) Patients who are low education level, no mobile phone or can not skillfully use mobile medical equipment.
