Inclusion criteria

Inclusion criteria: 1) Patients admitted in each center from November 1, 2018 to December 31, 2019; 2) Patients' age = 18 years old; 3) Vancomycin was prescribed by the physician.

Exclusion criteria

Exclusion criteria: (the marked item by * is only the exclusion criteria for the study of gene polymorphism) 1) Chronic dialysis patients; *2) Patients with chronic renal failure (CKD 4-5 stage); *3) Patients with acute renal injury before vancomycin treatment ; *4) Patients who have not signed the informed consent; 5) Patients who are considered unfit for enrollment by researchers; 6) Patients who are unwilling to participate in the study.
