Inclusion criteria
Inclusion criteria: 1. More than 65 years old; 2. To suffer from one or more of the following diseases: 1) Hypertension; 2) Coronary artery disease; 3) Type 2 diabetes mellitus; 4) Chronic obstructive pulmonary disease; 5) Stroke.
Exclusion criteria
Exclusion criteria: 1. Limited life expectancy(less than 1 year) due to severe or end-stage diseases; 2. Major disability or mental illness,Severe cognitive impairment, limited independent activity that cannot cooperate to complete evaluation; 3. History of malignant tumors.
