18 Years to 90 Years

Inclusion criteria

Inclusion criteria: 1. Aged > 18 years old and signed the informed consent; 2. At least one risk factor for heart failure; 3. Patients who are able to complete the review after discharge; 4. Agree that follow-up means are telephone (including WeChat), home visit, etc.

Exclusion criteria

Exclusion criteria: 1. Patients with acute coronary syndrome within 30 days; 2. Have a history of CRT implantation in the past 3 months or have plans to have CRT implantation at present; 3. Active myocarditis and constrictive pericarditis; Patients who expect a heart transplant or ventricular assist device within; 4. Patients who expect a heart transplant or ventricular assist device within 6 months; 5. End stage disease not caused by heart disease; 6. Failure to follow the research plan.
