Inclusion criteria
Inclusion criteria: Two types of patients with renal dysfunction 1.Meet any of the following criteria: 1) Augmented renal clearance (ARC) group:(50 patients for PK study and 50 patients for the group of dosing adjustment) a) Aged =50 years; b) Serum creatinine (Scr) 7 (age=50 years 6 scores; Trauma patients 3 scores; SOFA=4 1score); d) CLcr (8 hours)>130 mL /min; 2) Acute kidney injury group:( 50 patients for PK study and 50 patients for the group of dosing adjustment) a) Increase in Scr by 0.3mg/dl (26.5µmol/L) within 48 hours; b) Or increase in Scr to 1.5 times baseline, which is known or presumed to have occurred with in the prior 7 days; c) Or urine volume 0.5ml/kg/h for 6 hours. 2. Definite pathogen of infection. 3. Administration of hydrophilic antimicrobials. 4. The standard dosing of hydrophilic antimicrobials.
Exclusion criteria
Exclusion criteria: 1. Aged =18 years. 2. Renal replacement therapy. 3. Severe renal failure (eGFR 40 kg/m2).
