Inclusion criteria
Inclusion criteria: 1. essential hypertension,and the systolic pressure was measured greater than 140 at 3 times in a row in doctor's office; 2. Aged 40-70 years; 3. have smartphone and can use it in order to be followed by doctors; 4. the score of morisky is 4-6; 5. signed a contract with a family doctor at least for one year; 6. signing and willing to participate in this project, and giving consent to accept the diagnosis and treatment of hypertension at the designated medical institutions, and re-referred to superior medical institutions when necessary; will not drop out due to the long-term leave, going abroad or other reasons during the 1 year.
Exclusion criteria
Exclusion criteria: 1.secondary hypertension, hypertension emergency; 2. with type 1 diabetes; type 2 diabetes mellitus with acute metabolic disorder;secondary dyslipidemia patients (secondary to patients with type 2 diabetes not included); 3. acute myocardial infarction (<3 months) and stroke (<3 months) acute phase; 4. patients with chronic renal disease (eGFR <60ml/min/1.73m2 or urine albumin/creatinine ratio = 30); 5. pregnant women and nursing mothers; difficult to cooperate (psychiatric illness, hearing difficulty, or physical incapacitation); 6. to combine serious diseases with a life expectancy of less than one year; 7. healthcare provider; 8. participators in a similar project in the last 3 months; 9. data of blood pressure from doctor's less than 3 times.
