Inclusion criteria

Inclusion criteria: 1. Type 2 diabetes patients who have been diagnosed in our hospital and received continuing community care services; 2. HbA1c level > 7.0%; 3. Persons who have self-blood glucose monitoring instruments or go to community centers to measure blood glucose; 4. Those who have a certain level of education and can read the relevant content of APP; 5. Informed consent to this study.

Exclusion criteria

Exclusion criteria: 1. During the study period, the subjects quit, lost interviews, did not cooperate and died due to various reasons. 2. Insulin pump users; 3. Those with incomplete data; 4. Severe complications of diabetes mellitus; 5. Pregnancy or lactation; 6. acute complications and other serious chronic diseases.
