Inclusion criteria
Inclusion criteria: 1. aged 35-69 years Shanghai citizen; 2. diagnosed with diabetes (according to 2010 China guidelines for type 2 diabetes); 3. obtaining family support; 4. having more than one reversible risk factors (overweight or obesity with BMI=24kg/m2, central obesity with waistline =90cm in men or =85cm in women, hypertension with systolic blood pressure =140mmHg and/or diastolic blood pressure =90mmHg; 5. providing a written informed consent.
Exclusion criteria
Exclusion criteria: 1. presence of severe diabetes-related complications, chronic organic diseases, cancer, acute or chronic communicable diseases, motor dysfunction or mental disorder; 2. women in period of pregnancy or breast feeding,; 3. alcohol abuse or use of prohibited drugs; 4. poor compliance; 5. inpatients; 6. a history of serious cardiovascular diseases such as myocardial infarction, stroke, acute coronary syndrome and transient ischemic attack.
