Inclusion criteria

Inclusion criteria: 1) Interviewer: Informed consent, the main body of social support is related to the management of chronic patients practitioners, or the relevant government departments of chronic disease managers; 2) Subjects: Informed consent, literacy ability, in the hospital was diagnosed as chronic diseases, such as hypertension, diabetes, coronary heart disease, COPD, stroke, renal failure and other chronic diseases, at least one disease of adults and elderly people.

Exclusion criteria

Exclusion criteria: 1) Refusal of participants; 2) hospice caregivers.
