18 Years to 79 Years

Inclusion criteria

Inclusion criteria: 1) registered in local community health service centers; 2) with type 2 diabetes based on diagnosis by a doctor, for at least 6 months prior to study enrolment; 3) aged 18–79 years; 4) with HbA1c levels of =7.0 %; 5) without plans to leave home for the next 12 months; 6) Having a adult family member, who can use WeChat and living with the patient for a long time or visits at least once a week.

Exclusion criteria

Exclusion criteria: 1) Type 1 diabetes mellitus, gestational diabetes mellitus or other types of diabetes mellitus; 2) women who are pregnant or are going to be pregnant; 3) unable to complete 12-month follow-up; 4) unwilling or unable to provide informed consent; 5) unconscious or other serious diseases; 6) Participating in other diabetes intervention studies.
