Inclusion criteria

Inclusion criteria: 1. Aged = 65 years old; 2. Patients discharged from the Department of Neurology, Geriatrics/General, cardiology, and endocrine general wards in the first week of each month in March, June, September, December of 2017; 3. At least have one of five diseases including hypertension, hyperlipidemia, coronary heart disease, cerebral infarction, and diabetes.

Exclusion criteria

Exclusion criteria: 1. hospitalization time = 48 hours; 2. Multiple admissions within 1 month; 3. Transferred from or to the intensive care unit.
