Inclusion criteria

Inclusion criteria: 1. Biopsy-proven adenocarcinoma of the rectum; 2. BMI= 35kg/m2; 3. Rectal cancer located 4-12 cm from the anal verge: 4. Suspicious internal iliac lymph node or/and obturator lymph node involvement (The diameter of lymph node should >7mm before neoadjuvant therapy, or >5mm after neoadjuvant therapy); 5. Patients must have an Eastern Cooperative Oncology Group (ECOG) performance status of 0 or 1; 6. American Society of Anesthesiology (ASA) class I to III lesions; 7. R0 after resection; 8. After the evaluation of Multi-disciplinary team (MDT); 9. Written informed consent.

Exclusion criteria

Exclusion criteria: 1. T4b tumor that invade the external sphincter or levator ani muscle or neighbor organs; 2. previous abdominal surgery, no distant metastasis(except lateral lymph node metastasis) or synchronous colon cancer; 3. Recurrent rectal cancer; 4. Distant metastasis; 5. Obstructing rectal cancer; 6. Synchronous colon cancer; 7. Pregnant or breast-feeding women; 8. Fecal incontinence; 9. History of prior colorectal cancer; 10. History of inflammatory bowel disease; 11. Other malignancies diagnosed within the previous year.
