Inclusion criteria
Inclusion criteria: 1. Male or female, 18 years old or older; 2. Locally advanced renal cell carcinoma with level II - IV inferior vena cava tumor emboli (T2b - T2c) and without distant metastases; Hilar tumor, defined as a tumor located in the hilar region, preoperative imaging showed that the distance between the tumor and the renal artery or vein or the renal artery and vein was less than 5mm; Complete endogenous renal tumor is defined as tumor that is completely located in the renal parenchyma; 3. The patient was a candidate for surgical treatment (radical nephrectomy plus removal of inferior vena cava carcinoma thrombectomy) with a life expectancy of more than 3 months; 4. The predrug puncture was definitely RCC; 5. Did not receive other surgical treatment before surgical treatment; 6. ECOG status score 0 or 1; 7. The laboratory examination meets the surgical standards (the specific standards are listed in the case report table); 8. No surgical contraindications; 9. The patient signed the informed consent form and received treatment; 10. Low-molecular-weight heparin therapy and low-dose anticoagulation therapy are allowed to maintain the patency of central veins and prevent deep vein thrombosis.
Exclusion criteria
Exclusion criteria: 1. Patients with advanced tumor with cachexia; 2. Cannot accept preoperative puncture and cannot insist on surgical treatment; 3. Surgical contraindications (including bleeding and coagulation dysfunction, severe cardiopulmonary insufficiency); 4. Systemic systemic treatment or local treatment (including chemotherapy, immunotherapy, other drug targeted therapy, radiofrequency ablation, radiotherapy, cryotherapy or vascular embolization) before surgery; 5. In addition to kidney cancer, he/she has the concurrent merger of other active malignant tumor diseases or has the history of other malignant tumor diseases within 5 years (excluding actively treated basal cell carcinoma and cervical carcinoma in situ); 6. Confirmed HIV or hepatitis b patients; 7. Seizure or brain metastasis, spinal cord compression or meningeal metastasis (cancerous meningitis); 8. Serious uncontrollable disease or active infection; 9. Onset of any of the following within 12 months prior to surgical treatment: myocardial infarction, uncontrolled angina, coronary/peripheral artery bypass grafting, symptomatic congestive heart failure, stroke, or transient ischemic attack.Deep vein thrombosis or pulmonary embolism occurred 6 months before the operation; 10. Failed to meet the laboratory examination standards for surgical treatment.
