65 Years to 999 Years

Inclusion criteria

Inclusion criteria: (1) HK Chinese with Hong Kong Identity Card; (2) aged = 65 years old; (3) able to understand, listen and speak Chinese; (4) able to communicate (as assessed by the interviewer); (5) higher risk of hospital readmission.

Exclusion criteria

Exclusion criteria: (1) are psychiatric and mentally handicapped ; (2) live in institution.
