Inclusion criteria
Inclusion criteria: (1) patients with type 2 diabetes who meet the diagnostic criteria for diabetes: 7.5% to 9% glycosylated hemoglobin; (2) visited chengdu Ryan diabetes hospital regularly for one year and volunteered to join the diabetes care; (3) Aged 18 and 74 years old; (4) can comply with all research requirements, provide complete and authentic information and fill in the scale; (5) those who agree to participate in this study and meet the above criteria may be included in this study.
Exclusion criteria
Exclusion criteria: (1) the presence of surgery, trauma, pregnancy, infection and other patients may significantly affect the condition of diabetes; (2) serious mental disorders, consciousness is not clear or not cooperative, serious cognitive impairment, language expression is not clear; (3) complicated with acute and chronic serious complications: such as severe diabetic nephropathy, diabetic neuropathy, diabetic retinopathy, ketoacidosis, etc.) and complications: such as severe coronary heart disease, cerebral infarction, etc., patients who meet any of the above criteria will be excluded.
