Inclusion criteria

Inclusion criteria: 1) aged 65 years or above; mild cognitive impairment, mild and moderate dementia; 2) at least one disability in daily life activities because of cognitive dysfunction; 3) standing on or with aids; 4) living in the urban, can communicate with the Sichuan dialect or mandarin; 5) with a cohabitant, or who with them at least 4 or 7 hours per week; 6) consenting to take part in this study.

Exclusion criteria

Exclusion criteria: 1) patients are undergoing home-based rehabilitation (nursing, physical therapy, and occupational therapy); 2) a home modification within 12 months; 3) having an end-stage disease diagnosis (expected survival <1 year) or a treatment of cancer; 4) plan to move within 1 year; 5) patients are taking cholinesterase inhibitors (donepezil, galantamine, and carradine) or using memantine less than 3 months; 6) severe dementia.
