Inclusion criteria
Inclusion criteria: The subjects should be 35-74 years old and have been living the local community at least 6 months who agree to set up personal health records and receive health management. The subject is eligible if he or she has at least one of the 4 following risk factors: 1) Hypertension (SBP/DBP=140/90 mmHg or taking medication therapy); 2) Diabetes (Fasting serum glucose =7 mmol/L or taking medication therapy); 3) Dyslipidemia (TC=6.22 mmol/L, LDL-C=4.14 mmol/L, HDL-C<1.04 mmol/L, TG=2.26 mmol/L or taking medication therapy) 4) Obesity (BMI=28kg/m2)
Exclusion criteria
Exclusion criteria: The subject should be excluded from the project if he or she has any one of the following items: 1) stroke; 2) myocardial infarction or coronary heart disease; 3) severe cardiac insufficiency with NYHA III or IV; 4) aneurysm or dissecting aneurysm; 5) Atherosclerosis Obliterans (ASO); 6) chronic obstructive pulmonary disease or pulmonary heart disease; 7) secondary hypertension, thyroid diseases, hypothalamic-pituitary diseases, adrenal diseases or other endocrine diseases; 8) any acute problems to be treated such as acute infection, hypertensive crisis, diabetic ketoacidosis, etc.; 9) severe abnormal hepatic and renal function, autoimmune diseases, or other fatal diseases; 10) cancer; 11) being pregnant or lactating women; 12) mental disorders, alcoholism, acrasia, or problem with inexpressibility; 13) other incompliance problems such as metal impairment or physical disability.
