Inclusion criteria
Inclusion criteria: 1. Residents who have been contracted or are about to sign a contracted urban community and diagnosed with hypertension; 2. Diagnostic criteria for hypertension: mean systolic blood pressure =140 mm Hg and / or diastolic blood pressure =90 mm Hg, or who are taking antihypertensive drugs; 3. Aged at least 18 years old; 4. Communication barrier-free; 5. Voluntarily participate in the project and agree to complete follow-up.
Exclusion criteria
Exclusion criteria: 1. Patients with malignant tumors and life expectancy <3 years (as judged by community doctors); 2. Pregnant women or patients who are trying to conceive and have a birth plan; 3. Serious diseases with poor prognosis (as judged by community doctors); 4. Visual, auditory and intellectual disabilities, cognitive impairment; 5. Patients with severe depression.
