Inclusion criteria

Inclusion criteria: 1. essential hypertension; 2. high risk subjects with coronary heart disease: (1) at least have hypercholesterolemia or (and) diabetes. (2) have a combination of two or more risk factors as following: overweight and obese; smoking; lack of physical activity; male or postmenopausal women; family history of cardiovascular disease. At the same time meet the following three criteria: (1) permanent residents of the community; (2) signing and willing to participate in this project, and giving consent to accept the diagnosis and treatment of hypertension/hypercholesterolemia, diabetes at the designated medical institutions, and re-referred to superior medical institutions when necessary; (3) will not drop out due to the long-term leave, going abroad or other reasons during the 1 year.

Exclusion criteria

Exclusion criteria: (1) secondary hypertension, hypertension emergency; (2) with type 1 diabetes; (3) type 2 diabetes mellitus with acute metabolic disorder; (4) secondary dyslipidemia patients (secondary to patients with type 2 diabetes not included); (5) acute myocardial infarction (<3 months) and stroke (<3 months) acute phase; (6) patients with chronic renal disease (eGFR <60ml/min/1.73m2 or urine albumin/creatinine ratio = 30); (7) pregnant women and nursing mothers; (8) difficult to cooperate (psychiatric illness, hearing difficulty, or physical incapacitation); (9) to combine serious diseases with a life expectancy of less than one year; (10) healthcare provider.
