Inclusion criteria
Inclusion criteria: Clusters (units of randomization) criteria: 36 maternal child health units associated with one of six participating maternity hospitals. Patient criteria: Women between 20 – 42 years of age; Speaks Mandarin; Either a registered Shanghai resident, married to a registered Shanghai resident, or non-resident but has lived in Shanghai for 5 years; Able to read, understand and sign the consent form. Plans to deliver in a participating study hospital; MCHs unit where the child will be registered has to be identified and must be a participating SCHeLTI MCH. Inclusion criteria specific to the preconception period: Planning pregnancy in the next 6 months. Inclusion criteria specific to the pregnancy period: Singleton pregnancy; Less than 14 weeks of gestation at the time of enrolment. Fetal viability confirmed by ultrasound (positive fetal heart)
Exclusion criteria
Exclusion criteria: Clusters (units of randomization) criteria: Less than 100 potential eligible participants/year. Patients criteria: HIV positivity, measureable viral load of hepatitis B or C, hepatitis B core or e-antigen positive, Hepatitis C core antigen positive. Women with known major heart conditions leading to functional limitations or deficit, thalassemia major, cystic fibrosis, pre-existing type 1 diabetes mellitus, type 2 diabetes mellitus and/or chronic hypertension dependant on medication; Drug abuse; Planning to move out of the district within 5 years. Exclusion criteria specific to preconception period: Planning to conceive by in vitro fertilization. Exclusion criteria specific to pregnancy period: Known multiple pregnancy; Known major fetal malformation; "in vitro fertilization" pregnancy
