Inclusion criteria
Inclusion criteria: (1) Aged 65-85 years old; (2) Hypertension, hyperlipidemia, coronary heart disease, cerebral infarction, diabetes patients in Cardiology Department, cerebral infarction ward of Neurology Department, and Endocrinology Department; (3) Using more than 5 oral medications for cardio-cerebrovascular systems and diabetes; (4) Patients volunteered to merge and sign the informed consent.
Exclusion criteria
Exclusion criteria: (1) Patients with cancer, blood system diseases and other serious diseases or life expectancy less than one year; (2) Taking non-cardio-cerebrovascular or non-diabetic drugs such as respiratory and digestive systems medications at the same time; (3) Patients who stay in ICU; (4) Peri-operative patients; (5) Patients with hearing loss, mental retardation, severe visual impairment and other factors that affect the quality of follow-up.
