Inclusion criteria

Inclusion criteria: (1) aged 65-85 years old; (2) hypertension, hyperlipidemia, coronary heart disease, cerebral infarction, diabetes; (3) using more than five cardiocerebral vascular system and oral diabetes medicine; (4) patients voluntarily sign informed consent

Exclusion criteria

Exclusion criteria: (1) serious diseases, such as cancer, blood system disease, or other serious diseases, or less than 1 years of life expectancy; (2) patients admitted to ICU; (3) perioperative patients; (4) patients with deafness, mental retardation and severe visual impairment; (5) patients who were unable to use app or telephone; (6) patients with difficult communication; (7) move for the relocation.
