35 Years to 80 Years

Inclusion criteria

Inclusion criteria: 1) Permanent resident; 2) Aged 35-80 years and diagnosed by essential hypertension by qualified hospitals; 3) FBG==7mmol/L or HbA1c=7%; 4) Be capable of reading, writting and using a smartphone; 5) Has signed informed consent.

Exclusion criteria

Exclusion criteria: 1) Not with type 2 diabetes; 2) Patients with illiteracy, impaired basic reading and writing, inability to take care of themselves and mental illness or brain diseases; 3) Combined with severe heart and lung disease, cancer and other diseases are not suitable for wearing smart equipments.
