Inclusion criteria
Inclusion criteria: Patients with 1. Acute upper respiratory symptoms, fever, headache, muscle pain, pharyngeal pain and other clinical manifestations; 2. Chest radiographic examination revealed new patchy infiltration, leaf/segment consolidation, ground-glass opacities or interstitial changes, with or without pleural effusion; 3. Pharyngeal swab, sputum, bronchoalveolar lavage, lung or other biopsy materials showed virus nucleic acid test positive; 4. meet the diagnostic criteria of severe pneumonia: meet the following 1 primary criteria or 3 secondary criteria; primary criteria: Need invasive mechanical ventilation; Septic shock after active fluid resuscitation still requires vasoactive drug therapy. Secondary criteria: 1. Breath rates?30/min 2 oxygenation index ?250; 3. Multiple lobes infiltration; 4. Consciousness disorder and (or) disorientation; 5. Blood urea nitrogen? 7.14mmol/L; 6.Systolic pressure < 90mmHg and needs active liquid resuscitation.
Exclusion criteria
Exclusion criteria: 1. aged < 18 years; 2. pregnancy; 3. received glucocorticoid therapy in the last 3 months; 4. clearly diagnose the following diseases: (1) acute cerebrovascular disease; (2) acute coronary syndrome; (3) acute pulmonary edema; (4) acute asthma attack; (5) severe arrhythmia (as primary disease); (6) epilepsy; (7) drug poisoning; (8) burns or trauma; 5. the hemodynamic instability caused by the active hemorrhage of digestive tract; 6. clear immunodeficiency diseases or other clear immunosuppressive states; 7. need immediate surgery; 8. AIDS; 9. incomplete acceptance of standard treatment programmes.
