35 Years to 90 Years

Inclusion criteria

Inclusion criteria: 1. Diagnosed by hypertension or type 2 diabetes; 2. At least 9 years of education; 3. Aged 35-90 years; 4. The informed consent of patients.

Exclusion criteria

Exclusion criteria: 1. History of drug abuse or dependence; 2. pregnant; 3. serious physical illness; 4. mental illness; 5. brain disorders; 6. cancer; 7. language barriers; 8. serious complications of chronic diseases; 9. had participated in another self-management intervention.
