Interventions

Group A:quadratus lumborum block group: body weight (30- <50kg), injection 0.4% ropivacaine 20 ml, containing dexmedetomidine 20 micrograms and dexamethasone 5 mg
body weight (50- <75kg), injection 0.4% ropivacaine 25 ml, containing dexmedetomidine 25 micrograms and dexamethasone 5mg
body weight (75- <100kg), injection 0.4%
Group B:matched volume saline injection

18 Years to 75 Years

Inclusion criteria

Inclusion criteria: Patients undergoing unilateral total hip arthroplasty for hip arthritis or femoral head necrosis older than 18 years

Exclusion criteria

Exclusion criteria: ASA IV-V, with a body weight of 100 kg, allergic to topical anesthetics, study drug intolerance, psychiatric patients, topical skin infections at the puncture site, peripheral neuropathy, Clotting disorders, opioid abuse, chronic pain patients, severe liver and kidney dysfunction, low educational attainment can not use PCA or can not cooperate with VAS score
