Inclusion criteria

Inclusion criteria: 1. High-risk patients aged between 40 and 80 years who presented for high-risk groups screening at Lanzhou University Second Hospital, Gansu, China, in 2013 were enrolled in the study. (The high risk group contains at least three of the eight factors: (1) history of Hypertension (=140/90mmHg) or taking antihypertensive drugs; (2) atrial fibrillation (AF) or arrhythmia; (3) smoking; (4) Dyslipidemia; (5) Diabetes; (6) Rarely sports; (7) Overweight; (8) Family history of stroke; 2. Have the possibility to complete 24 months follow up including telephone conmunications and questionnaire surveys; 3. Would like to test weight, blood pressure, blood lipid, HbA1c regularly; 4. Reading ability; 5. Accept the Pharmaceutial care research of own free will; 6. Patient is informed and has given written informed consent.

Exclusion criteria

Exclusion criteria: 1. History of stroke and TIA; 2. Communication barrier; 3. Severe cardiovascular disease, severe respiratory disorders, severe liver and kidney dysfunction and malignancy.
