Inclusion criteria

Inclusion criteria: 1. Spastic hemiplegia following different causes of central neurologic injuries (CNIs), such as stroke, traumatic brain injury, cerebral palsy, majorly presenting difficulty in hand-open; 2. With normal mental and linguistic functions preserved; 3. Aged 12-45 years old, no limit of gender; 4. The patients had received regular rehabilitation therapies for at least 5 years and ceased improving in the paralyzed hand; 5. The patients (or their parents if the patients were underage) were willing to receive surgeries to achieve further functional improvement; 6. Brain MRI scans verify that central neurologic injury only involves the hemispheres contralateral to the paralyzed hand; 7. The paralyzed hand was controlled by the ipsilesional hemisphere exclusively, indicated by the transcranial magnetic stimulation (TMS).

Exclusion criteria

Exclusion criteria: 1. With severe systemic diseases such as diabetes mellitus, cardiopulmonary diseases that were unable to tolerate the surgery; 2. For personal reasons of patients or the family members urge termination of surgery; 3. Patients who were psychologically or mentally abnormal; 4. Patients who mainly present ankyloses of joints; 5. With severe fixed contracture or joint deformity; 6. Mental retardation or poor learning and communicating capacity (relative contraindication).
