Inclusion criteria

Inclusion criteria: Persons aged 50 to 79 years old who resided in the two communities longer than 6 months and were willing and able to participate fully in all procedures of the program and follow-ups were eligible for inclusion.

Exclusion criteria

Exclusion criteria: Exclusion criteria were suffered cardiovascular events needed for hospital treatment, had chronic kidney disease, diabetic foot, secondary hypertension, active cancer or mental disorder, with visual or hearing impairment, planning to leave the area prior to the anticipated end of participation, or participated in another clinical trial currently or within the previous 6 months.
